Using Radio Buttons instead of checkboxes

I have the following from a webfacing conversion:

This creates two checkboxes along with their labels.  I need the selection to 
be exclusive so I have to use radio buttons.  Any suggestions on how to do this 
as I am not too familiar with javascript.  Thanks.
