If this is strictly a CHAIN using a logical file then the first record
returned should be the duplicate with the (currently) lowest relative
record number. Under the covers the system includes the RRN of the record
in the key, generally right after the actual key values. Note that
"currently" is important due to reuse of deleted records.

Thanks Kyle and John.

As is usually the case, once I see the SQL it's like "Oh, that makes
sense."

select InvoiceNbr, ItemNbr, count(*)
from myTable
group by InvoiceNbr, ItemNbr
order by count(*) desc ;

Another curiosity question in this situation.

If I wanted a count of the number of invoices that have an item
appearing more than once, how would I do that in SQL?

I simply don't know enough to know what to google. Though I'm
thinking
the count function will be in there twice . . .
