first of all: I don't use SR since procedure came up.
In most cases open and fetch are in diffrent procedures, so the declaration should be global, it's better for readability (and the SQL compiler anyway).
Good old Pascal style says: all declarations at the beginning of it's block => variant 3 is best choice.
