Your link appears to be an HTML table that someone copy/pasted from the
Information Center. It's not a copy book (unless I'm missing something)
and provides nothing that isn't in the manuals.
Personally, I'd suggest using the one in the manuals, since IBM usually
does a good job of updating it when new stuff is added in each release:

But, as far as Mike's question about a copy book -- I don't know of one,
and that's too bad, as it'd probably be useful.

I also at one time got a snippet of source code from Jon Paris, I think with SQL state codes in a data structure but Jon would have to post it for you.
