Look at calls that will have occurred in the statement execution between the successful chain to the file and the unsuccessful chain to the file. Go over them "with a fine toothed comb" looking for a longer length for an updated parameter in the called program than in the calling program. This is usually the culprit, expanding a little on what Scott said.
