You place it in the D-Specs. Here's some sample code. The trick is to make sure the extpgm is the name of your program. I place a comment above it so anyone searching for *ENTRY or PLIST will find this code.
Actually, unless you're CALLP-ing the prototype, it doesn't make any difference what the EXTPGM is. You'll get a warning message if it doesn't match the name of your program, but that won't stop it from working.
EXTPGM is only used when you actually use the prototype to call the program.
