When does the program that causes the trigger to fire do a commit? Is it
possible that the same record is updated, causing the trigger to fire multiple
times for the same record, without having done a commit before hand?

Does the process that fires the trigger also do I/O to the file the trigger
program is doing I/O to?

Are you saying that your trigger program does I/O to the file that the trigger
program is attached to?
You can't do that.
What are you trying to accomplish?
