Jon,
The title is Programming in RPG IV Third Edition. Yes it does
SubProcedures
and Freeform. It was my text book at NC State and I have it open on my desk
as I type this.

geared towards "skill enhancement" rather than introduction. While this
makes sense for their target audience, I wonder if there isn't a need for a
tutorial on RPG?

Actually we have taught some introductory classes Joe - mostly for COBOL
programmers but also for Java folks. In fact we have been thinking of an
"RPG for Java programmers" type of thing <grin>

There is at least one RPG introductory book - which the Judy Jaeger/Bryan
Meyers book from Penton. I know that one has been updated to include
subprocedures, it may also have free-form in the latest incarnation. It is
the basic text used by many of the colleges that teach RPG.
