I am at V5R3 as of last weekend.  When I attempt to read data into the
data structure, as in the example, when using EXTNAME for the Data
structure, the error states that the DS is not allowed for the
operation.  But if I use LIKEREC, the read works.

Interestingly enough, I also solved my last problem, simply by
incrementing the BASED data structure is based on by three bytes (the
size of the extra field).
