Not sure how much help this will be, other than to save you from pulling out
your hair.

For a lot of people, myself included, Microsoft "broke" Windows Defender
with the symptoms you described.  Too bad, because it seems to be a pretty
good product otherwise.  I never had problems with the version M$ had before
they renamed it to Defender, but I think I got Defender to update a few
times before it "broke".  I gave up after a week of checking for a fix; that
was several weeks ago, so I am somewhat surprised to hear that they still
haven't fixed it.

FWIW, there are a lot of other products that do what Defender was supposed
to do, some good ones even do it for free.  I have had good experience with
"Spybot - Search & Destroy" and "Ad-Aware".
