The earliest manual I can find online is from 1994, V3R1 of OS/400 - yes, those manuals are still online. You can find lots of older stuff at http://publib.boulder.ibm.com/cgi-bin/bookmgr/LIBRARY

The V3R1 manual says that referential integrity and long names were some of the new stuff at that release. So there had to be earlier stuff. I'm pretty sure I was exposed to it in 1991 or so, not sure the version. I saw a reference to V2R3 in a V3R1 manual.
