Try looking at the plan cache. It’s highly likely that an old plan cacheGood suggestion, but it didn't seem to make a difference.
is still influencing the results.
Remove the plan cache and see if it clears the issue.
I know, I know, any change to the SQL etc should update the plan cache,
but is adding an index without changing any of the other items in the SQL
going to generate a new plan cache? Force a change, then review the
results….
