It automatically reinstalled LIC when I ran an IPL-D, I didn't get any prompts or options. I can't seem to find initialize option :) After it did the IPL-B (A or B? I don't remember, it was automatic), I was back in my system with a standard login screen.

I'm pretty sure you forgot to set the IPL to manual. D alone isn't enough.
