I would also ask if these are public web servers. I know I get hit at
least once a week with DoS and SQL Injection attempts that really don't do
much but slow things down, but who knows.. maybe there's an apache issue
out there that someone found a way to take advantage of?

I would say if it doesn't happen again I wouldn't fret over it. If it
starts happening a lot, then open a ticket with IBM.

This is probably deep within the bowels of Apache and very likely hard to
impossible to track down. If you don't expose some web UI from the machine
to the outside world (be it directly or through a reverse proxy), I'd not
bother.
