Got this from IBM Support a couple of years ago

STRSST

Choose these options:

1 Start a service tool <enter>
4 Display/ Alter/Dump <enter>
2 Dump to printer <enter>
2 Licensed Internal code (LIC) data <enter>
14 Advanced analysis <enter>
On the first (blank) line, enter option 1 and enter "installhistory" (no
quotes) <enter>

This will create a QPCSMPRT spooled file - search the spooled file for VxR
(V5R, V6R, V7R, etc.) and it will show version/release number followed by
install/upgrade date a couple of lines below. There will be 2 entries for
the current OS version installed, the first hit on page 1 is information
related to the version, there will be another hit that contains
install/upgrade date further down the report.

There may be other ways to get this information - this is the one that I'm
aware of.
