Unfortunately we don't audit spooled files. I also don't know when it printed. All I know is that it was between Friday 5pm and Monday 8am.

Then that seems to imply that some job, not running under QSYS, is trying to open that file with write permissions.
If you're auditing spool files (we do) then you should be able to determine the job which generated that spool file.
The job name and user should help IBM determine a source.
I printed a spool file generated from job ROBHQ1 from user ROB to printer PRTL001.
