Is the barcode label correct? Tape in the correct position?
If both true, then most likely bad cleaning tape.
To show it's not a bad cleaning tape, manually mount it and see if it gives
you the same error. If it does not cause a problem replace the barcode
label (just for grins if no other reason)
After that, a call to IBM (assuming maintenance on the tape library)

The new cartridge is not being recognized and throwing errors.
