I've upgraded firmware on my Power 8 8286-41A.
I upgraded from
860.31 (109)
to
860.40 (118)

I have two VIOS lpars. One of the two started just fine. The other one
does not. It has a status of "Open Firmware" and a Reference Code of
AA00E1A9.

Google says that's because the LPAR booted into SMS mode and I should look
at the terminal.
We used to be able to open vios terminals through the HMC web interface
but, while it looks like the capability is there, I can't get it to work.
So I used the KVM interface to get to the HMC

In the failing VIOS lpar it detects no disks. Both VIOS lpars use their
own pair of mirrored disks located in the CEC.
Both VIOS lpars serve up Fiber Channel.
Both VIOS lpars serve up Fiber ethernet.
So we should be good on FC and ethernet redundancy.

On the HMC the good vios lpar uses

So, let me guess,
either C14 fried,
or the firmware update things it did.

C14 is an internal card and not one you can see from the back of the
machine.

Of course, this machine is at the data center in Grand Rapids.

I had no such issue on the other 41A I upgraded to this same firmware.

Note: this same firmware is loaded on to my 8286-42A waiting for it's IPL
this weekend.
