There are PTFs that no one knows about until you describe a problem and IBM says, "Oh, yeah. We fixed that a year ago. Here's the PTF you need." To be honest, they're usually for things like "if it's the 3rd of June and the moon is full and Jupiter aligns with Mars."
The cumulative only shows the things that affect lots of people; fixes that everyone ought to have.
Then there are the 'test' PTFs - things the folks in the lab think will fix a problem, and the code is reasonably stable, but they need more users with the problem to try the PTF and see if the patch is stable enough to release.
