Well, opened ticket with IBM.
They had me reinstall the OS.
I tried to do it from NFS (seriously) but I was having issues so I gave up
and did it from my VTL.
Of course that died because apparently when you start "current
configuration" from your HMC it happened to forget about the latest SCSI
client adapter I dynamically lpared to it and I lost 6 disk drives. No
problem. I powered it down again (by this time I had two IBMers on the
call with a remote session going...). I modified the config and added the
adapter and started the lpar again. This time I got past the disk drive
issue and got the base os restored.
And I reinstalled PTFs (from NFS) with no issue this time. Granted, the
system was in restricted state this time.

Ticket was closed with no possible further analysis as to why this
happened.
I did make a comment about "... happens, after all it's only IBM i,
right?"

One of the people from IBM was a bit baffled why "current configuration"
did not have the latest dynamically added adapter and will pursue that
independently.

Good thing that lpar was expendable. Now to try it on my biggest ERP lpar
and see if the users notice this one...
