I use *DLT per an earlier suggestion you posted here some years ago.--
Right now I'm re-initializing said tape with CLEAR(*YES) so that I can
trash it!

It depends. Do you use VOLSTAT(*DLT or *KEEP)? If you're using *DLT
and you're getting that many errors on a weekly basis - nuke it. But
if you use one tape (foolish) every night for the last 15 years and
you're using *KEEP, your opinion may differ.
I'm also guessing, since you're running V5R1, that your tape drive
isn't exactly 'leading edge'. IOW, used tapes for that should be a
dime a dozen (figuratively). Just replace them.
