PS-- if you already have a form you like, you can use a scanner and import a printed copy to your PC, tweak it in your graphics program, then print with IBM's print driver. The main issues are that you have to line up the original carefully to make sure any lines are really horizontal and vertical, and you tend to lose resolution on any text. I've done this with several overlays, and ended up re-keying the text and re-drawing some of the lines to sharpen things up. But it's a lot easier than designing a form 100% from scratch!

Best advice for using a scanner-- use the highest resolution you can-- the more bits the merrier! You can always toss away excess bits when you print, but you can never create more than you had to start with!
