For hosting I like IX web hosting (http://murl.me/ix affiliate link). Uptime has been good and every minor downtime is posted to twitter. I haven't used phone support, but chat support is speedy.

For domains, I use GoDaddy and haven't had a problem, but there is way too much stuff going on all of their pages. It's a nightmare figuring out anymore.
