The v5r4m0 to v5r4m5 upgrade is (AFAIK) a 'hardware enablement' release,
designed to provide the 'drivers' for the new hardware. As such, some
of the hooks into the OS were modified.
I refer you to the "IBM - Installation, Upgrade, Migration, and Recovery
- Quick Reference Chart." Unfortunately, I don't have a link; we
finished our upgrade last November and all I have is the printed copy in
my "How To Do It" manual.
In the "Upgrade of Modification level Only" section is a row for "v5r4m0
to m5r4m5." The Guide referred to is "System i: i5/OS and related
software Installing, Upgrading, or Deleting i5/OS and related software
(SC41-5120-09)." See Chapter 9: "Replacing Licensed Internal Code and
i5/OS of the same version and release."

It requires your "999" (microcode) CD, plus volumes 1 and 2 of the OS to
complete the installation.
