I didn't go to 6.1.1 from any 5.4.5 - only 6.1.0 but I think you need
SI35849. I too had a similar ptf that I had to put on to upgrade.
Try RMVPTF and DLTPTF. Then try to order it again.

For my ticket, going from 6.1.0 to 6.1.1, I too had the GO LICPGM work but
the VFY fail. In my pmr 12857,500 IBM posted
INFO APAR II14310. Missing PTFs SI35186 and SI35189.
