Overlay not showing sometimes

We put an electronic signature on some Bills of Lading. The entire printer
file has an overlay, plus we put the signature in a little box at the
bottom.



This has been working well for several years. We needed to create some new
signatures, so I followed the steps I always do. Create the jpg, print to
the AFP 300 printer driver (specify medium overlay), put the file in QDLS,
then do the Create OVL

The overlay prints on a test form. BUT I can't get it to print in our
application.

I've confirmed via debug all of these field values are correct. I don't
mess with overlays much. Has anyone seen anything like this before?
