As for the DBA, we don't have one and they won't be adding one.
So it'll be up to the programming staff to decide when to add them and
when not to.

Pick someone to play that role. Remember, you never need an index to get
data, just to get data efficiently. If this is new to everyone then they
will want all sorts of indexes since the needed them with classic IO.
There are some obvious indexes (Customer# on the Order table, Order# on
the OrderDetail table, probably item# on the OrderDetail table, etc.)
that you'll have. But don't go too crazy until you look at visual
explain and understand how the query is working.
