IBM ships a QAQQINI in library QSYS, but by default, the system tries to look 
for the QAQQINI file in QUSRSYS.  Using the CHGQRYA QRYOPTLIB() allows you to 
specify a specific version of QAQQINI in another library....

If you have never checked, WRKF *ALL/QAQQINI to see if the ini file is in 
QUSRSYS.  If not, you should use CRTDUPOBJ to copy the QSYS version to QUSRSYS.

Actually, this is a J2EE system deployed to WAS 6.0.2.11 on I5 V5R4. 
Data Access is through jt400.
I haven't done anything that would lead to the creation of a QAQQINI
file that I know of.

Also, it's working fine on one system and not working on another.  I
think that we're just going to contact IBM.  My guess is that it's a
crazy config that we just can't find, or that it's a PTF problem.

Not on your request list but, maybe I can help...

Are you using an user-defined QAQQINI file? If so, how was
it generated? Please be aware that IBM warns again copying
this object with any other method than a CRTDUPOBJ, due to
the triggers attached to the file.

In searching the archives, I see that a problem was
