You may want to try Adobe Acrobat, which is a tool to manuipulate PDF
files.  You should be able to, whether by script or manual methods, merge
the multiple individual PDF files.

Note -- Adobe Reader can only read PDFs.

Also -- perhaps you can get the DOC1 package to generate all of the forms at
once ...
