Reason codes for Access Plan not Updated - help please!

Can anyone point me in the direction of reason code meanings for field QVC24
(Access plan not saved) in record 1000 of the Database Monitor file???
It used to be Appendix D of the Database Programming Reference Manual, but
the V4 one doesn't list these particular reason codes and the new one
doesn't have an Appendix D!
