I'll throw in a few:

Countrywide Home Loans

Bushnell (which now includes Serengeti, Bolle, Tasco, H2Optix)

and last I heard, AS400 was the official computer for school systems in
North Carolina

on a more regional note:

Catholic Healthcare West

MGM (some royalty processing software -- my first excuse to use RPG IV)

NME (National Medical Enterprises -- are they still in business?)

or a worldwide note (but not in the U.S. anymore):
