The implication is that you replaced the bad drive and the IPL did not
report a failed drive that time?  So, now, the system has no "failed"
drives?

If so, seems like you need to call IBM on this and get them cracking.  Jeez,
I wonder if this will fall between the hardware service and software service
cracks?  Software service: "It's a hardware problem."  Hardware service:
"It's a software problem."

Is this a live production box?
