How do I create an overlay?

I have a Word2000 form that I'm trying to use as an overlay on our 400
(unsuccessfully). Searching thru the archives I came across the following:

CRTPF FILE(library/file) RCDLEN(8201) LVLCHK(*NO)
CPYFRMPCD FROMFLR(folder) TOFILE(library/file) FROMDOC(pcfilename)
TRNTBL(*NONE) TRNFMT(*NOTEXT)
CRTOVL OVL(lib/overlay) FILE(library/file) MBR(member)

When I do the CRTOVL command, I get the following error on the 400:

File DOC8130 in WEBTSTSRX not valid for creating a *OVL

Expanded help gives the following:
   2 - The structured field identifier in the incoming data for the printer
 resource is not correct or is not supported. The error was detected while
 processing record 1, byte number 0. The structured field which is not
 correct may be caused by a length that is not correct on the previous
 structure field. The structured field which was being processed at the
time is 'D0CF11E0A1B1'X.

What am I doing wrong or leaving out?  How _should_ I be creating an
overlay?
