I don't recall what the cume level was before upgrading.  It wasn't the most
recent.

There is a manual that comes with the upgrade.  In it, it tells you what you
need to do when upgrading from various releases.  I've done a lot of
upgrades over the years and have had no issues when following that manual.
(Well, when CD's first came out I put the wrong CD in the drive and IPL'd.)

Normally, I'd expect TLyyddd entries to tell me which cume level is applied.
Is that what the REyyddd entries are on V4R3?

Also, you mentioned the hardware model number.  Does that have any bearing
on
upgrading to a new release?

I've upgraded a 170 from V4R3 to V4R5 with zero problems.  I needed to apply
no PTF's prior to upgrading.
