A service program I wrote for HTML output uses API CEELOCT. I've had no
issues with this until we upgraded to V4R5 last weekend. Now, some programs
calling this service program fail with "The call to HTMLOUTINI ended in
error (C G D F)." The job log looks like this:

Tried to refer to all or part of an object that no longer exists. [MCH3402]
Unexpected user error occurred in CEELOCT. [CEE9902]
The call to HTMLOUTINI ended in error (C G D F). [RNQ0202]
? C
Application error. CEE9902 unmonitored by SPHTMLOUT at statement
.....

Recreating the service program does not seem to help. Surely the CEELOCT API
hasn't changed *that* much. Under which conditions would this happen?
