It's a disk or tape failure reported by the MFIOP(917B). Get the SRC's for 
13-2 and 14-2
take the right most 4 of 13-2 and the left most 2 of 14-2

The format will be BBCb unit#. Loadsource would be 0010 01, disk #2 0010 
02 etc in the SystemUnit.
If in the ExpansionUnit address will be 0050 01, 0050 02 etc.

Try a manual IPL and scan the disk or try the "A" side to manual and scan 
the disk which ever might work.
If you can't manual IPL try a "RESTORE" of LIC and then a manual IPL etc.

Does anyone have the Problem Analysis book for a model 200?  If so, would 
you please look up the above mentioned Source Reference Code for me.
