I have been a participant in contributing to this, although the person who 
got locked inside used phone to call someone who had key to come let them out.

We have a fire door between factory & offices.  In the evenings we sometimes 
have  a second shift in the factory, but staffing in the offices is vastly 
depleted, so a rulling came down that in this scenario we are to lock the 
firewall door separating factory from office, like when there is only one 
person working in the office late at nite.  There was concern about possible 
pilferage when factory workers took their breaks.

The head supervisor for factory workers had key & I had key to that door.
After everyone has left for the day, I do a check that doors locked, lights 
out, blinds drawn, etc. but unbeknownst to me, they had started using one of 
the factory workers to do janitorial work & he started coincidentally on the 
night that I left early ... when I did my quick walk around the office to 
make sure no one in any of the offices (some of my co-workers do not have the 
keys), the new janitor was in the executive wash room.

The janitor did not know how to work the intercom system so he had to wait 
until it dawned on the factory supervisor that the janitor person was 
missing, which was when the supervisor was checking parking lot to figure out 
whose car that was ... sometimes people have car trouble & are slow getting 
out.

We have a chain link fence that goes all the way around the property & a 
number of gates that are open during the day.  The factory workers leave an 
hour before the office workers, then after they are gone, someone locks up 
all gates except one & then the last office worker to leave (usually me) 
locks the last gate.  When the mob leaves there are these gates wide open so 
they unfamiliar with practices of the final departees.

One night some folks had auto trouble, got ride from a friend to a gas 
station, had the service folks ride them back to their car & they were busy 
working on it when I left.  I did not see them because they were on the other 
side of the building from where I was leaving.  I locked the gate like usual.

When my co-workers had their car problem solved, they drove around the 
building to the gate they had come in by only to find it locked.  Fortunately 
someone had a car phone and knew #s of co-workers so they could find someone 
who had a key to come let them out.
