Directory browsing failed - access forbidden -- MIDRANGE-L

Recognize this one??  I've been wrestling with this one, trying to set
up multiple configurations and instances.  So far, I've been able to
somehow get past this, but on this last config, I cannot.

And, what is the correct order of
1. configuration creation
2. instance creatioin
3. cfgtcp stuff
4. start tcp for this address

So far (except for this last config) I've been able to get it working,
but likely never in the ?correct? order.
