We are using Optio Software. We have created more than 70 forms that are
currently in production, including A/P Checks which we also print the
signatures.  The software has alot of functionality, and it's fun to use (if you
can believe that!).  We are using HP printers with PCL-5 emulation.  You could
create your form exactly, including logos, shading, boxes, lines, you can map
the spoolfiles to print in different fonts.  It's really cool....
