Or the team of electricians who whilst moving some desks 'terminated' some dumb
terminals by severing the cables with cable cutters.  Lost a whole port on a
twinax controller.

How many people have received a call, usually early on a Monday morning, that
starts, "We cleaned up
the office and moved around some of the terminals and now none of them work."
Or, "Our terminals
won't work anymore"

"Did you do anything to them?"

"No"

"Did anyone disconnect any wires?"

"Oh, yeah, <Insert name here> didn't like his/her desk so we moved her across
the room.  She took
her terminal with her"
