We noticed the same problem when starting up our emulation sessions
after going to V3R1M3 and applying the latest service pack.  It seems
that we had to recreate the Icons, becuase the existing Icons were
calling something that had been replaced.  The funny thing was, we only
had to replace the icons on some workstations.  

While it is not an answer, it might be a good lead.
