The request "graphics/banner.gif" is a relative file that is retrieved from
whatever directory the server is in at the time the file is requested.  Put in
a / before the graphics/banner.gif.  Make sure the "/graphics" will hit a Pass
rule.

The client may put in additional path information when / is not specified or
your configuration may need to be configured with or without a leading slash on
the Map, Pass rules.
