The answer from Intellichief is that they do not offer this form.
I will create myself, I do love using the software so it's just a matter of time.

We did purchase the XA forms pack along with it. I have a support ticket in now with them raising the question.

We own the Intellichief forms package Formtastic. I can't believe, since both are integrated with XA and under the umbrella of Infor, that there isn't a template with Formtastic, but there isn't one.
