With respect to question #1, by default the web.xml is read from the [instance_root]/config/cells/cellname/applications/applicationname.ear/deployment/applicationname/.... directory structure. Not from under the installedApps directory structure. If you change the web.xml file under the config directory structure for the Express instance, you should only need to restart the application to have the changes take affect.
