And would depend on your definition of "hacked." Most break-ins are the result of user security error. I worked for a place where the manager would often sign on to the POS terminals to correct mistakes made by the sales reps. One day while he was in my office and I was reviewing the system activity I noticed he was also signing on and off across town to "correct" another mistake. The problem was, of course, that he had too often signed on in the view of operators who could make a profit using his authority. I'm sure there are about a million such anecdotes.
