Dialing Prefix & Faxupdate

Need to change the dialing prefix for our fax.  We are running Lotus Fax
for Domino V4R1M0 on I-Series w/V5R1.  Domino is at release 6.01

If I go to the server config & click on the fax tab it is blank :-(

I tried to run the fax setup again & it is failing because of this:

Adding device form/subform:  $LotusFaxDTMFRoutingWithDIDAS400
Error copying design element $LotusFaxDTMFRoutingWithDIDAS400
Error :  Entry not found in index

This is really starting to drive me nuts!!!!
