One of two things that I can think of.

a) The program wasn't using the service program. You can check this by doing
a DSPPGM and paging (<Enter> key) through to the service program list.

b) The AG in which the service program was running has not ended.  e.g. The
service program runs in *CALLER and your program ran in QILE.  Unless you
delete the AG or sign off, QILE remains, and the service program remains
active.  Even if you delete the service program it is still in the
activation and will be picked up by the next program that wants to use it.
