I have built an add-on similar to what you describe.
It tracks changes to critical BPCS files (add, change,
delete) and logs date/time/userid along with the
changes made.  It can also pop up a verification
window which requires the user to key in their AS/400
userid and password if an electronic signature is
needed (such as with fields that have
regulatory-agency impact--FDA, DEA, etc.)  The main
purpose of this add-on was to maintain an audit trail
of "key" master files, such as Lot Master, Item
Master, security files, etc., but the same concept
could be applied to transactional files.

This add-on, as it is based on triggers, will also log
changes made "outside" BPCS, such as someone changing
a file directly through DBU or SQL.
