You might check out that the items involved are coded correctly. We have had intermittent problems with items coded as purchased that should be coded manufactured, or new employees unfamiliar with our number systems looking at BOM & releasing a shop order on a purchased item. Apparently if items are keyed in with human error not noticed immediately, the system accepts as valid & does weird stuff.
