Thanks for all the ideas so far but still no joy. The machine is a ways back on PTFs (V5R2 June of 04) so we'll be getting that corrected but at this point still no includes....

Besides the +Includes option, I have the following:

AddOutputFilter INCLUDES .shtml
AddOutputFilter INCLUDES .htm
AddOutputFilter INCLUDES .html
AddOutputFilter INCLUDES .pgm

My HTML pages have things like <!--#include virtual="/navigation1.htm" -->
in them. I see that you have "include virtual" in uppercase. Have you tried
making it lower case? Also, if you're on V5R1 or V5R2, make sure that you've
installed the group PTF's for 5722DG1. There were problems with SSI's in the
initial release of the server.
